OCCUPATIONAL ACCIDENT INSURANCE........
- Beginning in September 2010, Ace Doran will initiate an Occupational
Accident Insurance program. The policy will be through Zurich Insurance , and it will be administered
through First Guard Insurance Agency. The policy offers the best available coverage and top level benefits
at a competitive price. We will begin offering this insurance in September, and we will require all leased
owner operators to participate by December 2010.
FLATBED CONTRACTORS-DRIVERS........
- 23 Years Of Age
- 3 YEARS Over the Road & Flatbed or Equivalent Experience Desired
- CLASS A CDL Issued by State of Residency
- SATISFACTORY DRIVING RECORD (Subject to Review)
- 3 or fewer violations within the past 3 years
- 1 or fewer preventable crashes within past 3 years
- NO preventable DOT Recordable crashes within past 3 years (No upsets, rollovers or rear end crashes)
- NO serious cargo loss in the previous 3 years
- NO DUI within past 10 years
- Acceptable Roadside Inspection Record (It will be checked)
- Current Physical (Within past 12 months) & acceptable Body Mass Index
- PASS A PRE-EMPLOYMENT DRUG TEST- Anticipate the applicant to have participated in a DOT compliant drug program during the previous two years and to have been in compliance for the past 10 years (Subject to Review)
FLATBED EQUIPMENT..............
- Tractors and Trailers Must Pass a DOT Annual Level Inspection before leasing
- All Inspections are at Ace Doran Inspection Vendor Locations
- All equipment over 10 years old will also be inspected in Cincinnati
- Trailers must have good floors and have full conspicuity tape package installed)
- Cargo Handling Equipment (Suggested): Twelve Chains and Binders, 12 to 14 Straps, chain and strap protectors, TARPS, 8 coil racks, padding for tarp protection, load levelers for drop deck
FLATBED EQUIPMENT INSPECTIONS and MAINTENANCE
REPORTS..............
- VEHICLE INSPECTIONS are due EVERY 90 DAYS (to occur during that month). ACE DORAN has inspection locations conveniently located.
- MAINTENANCE REPORTS are due MONTHLY. Sent to the Safety Department in Cincinnati.
DOT DRUG TESTING..............
- DRUG and ALCOHLOL TESTING Pre-Employment/Random/Post Crash
- Contractors/Drivers will pay $53.00 for the INITIAL PRE-EMPLOYMENT test. A FEE of $35.25 will be charged ANNUALLY for Random and Post Crash Testing. This charge will be deducted through the 10% RETENTION

- Copy of your long form physical
- Copy of your medical card
- Copy of your CDL- (PLEASE HAVE CDL ENLARGED 114% BEFORE FAXING)
- ALL QUALIFYING CANDIDATES MUST ATTEND AN ORIENTATION IN CINCINNATI WITHIN 2 WEEKS OF SIGNING LEASE.
- IF YOUR APPLICATION IS FAXED YOU ARE STILL REQUIRED TO SEND THE HARD COPY OF THE APPLICATION AND ALL REQUIRED FORMS TO THE SAFETY DEPARTMENT IN A TIMELY MANNER.
